Jay Lee, MD, MPH, FAAFP
Family Physician • Executive • Change Agent
Dr. Lee is a proven physician leader and vocal advocate for accessible, equitable, and high-quality health care. A family physician, executive, and change agent, Jay serves on the Board of Directors of the American Academy of Family Physicians (AAFP), representing over 128,000 physicians and medical students nationwide. In this role, he works to drive transformation in our health care system, positioning patients and front-line physicians at the center of reform.
Jay is Medical Director at Integrated Health Partners of Southern California, a clinically integrated network serving over 350,000 patients whose medical home is at one of nine community health centers in three counties. He continues to care for his patients at a local community health center and is an associate clinical professor of family medicine at the University of California, Irvine.
Throughout his career, Jay has held physician executive roles that focus on making health care work better for everyone – leading practice transformation at a vertically integrated health system, innovating primary care model at a national value-based care organization, and overseeing day-to-day operations at nationally recognized community health centers in Los Angeles and Orange counties. His experience spans policy, practice, and people.
A dynamic AAFP member since 2002, Jay has served as a delegate to the Congress of Delegates, as 2012 convener for the National Conference of Constituency Leaders, as a member of the Commission on Health of the Public and Science, and as core faculty for the Leading Physician Wellness program. He co-founded the Family Medicine Revolution movement (#FMRevolution), a grassroots social media movement giving the power of telling family medicine’s stories back to family physicians and building a global community of thought leaders. He was also a member of the inaugural class of media ambassadors.
In California, Jay is a past president of the California Academy of Family Physicians (CAFP) and chaired the Family Physicians Political Action Committee. CAFP honored him for his inspirational leadership by awarding him “Hero of Family Medicine” in 2018 and “Family Physician of the Year” in 2021.
Jay earned his bachelor’s degree in human biology with departmental honors from Stanford University. He then worked for a non-governmental organization in rural post-war El Salvador, where he supported local physicians in a mobile clinic and managed a potable water project. Returning stateside, he graduated as a dean’s scholar from the Keck School of Medicine at USC then completed his residency at the Long Beach Memorial Family Medicine program. He earned a Master of Public Health in health policy and management from the Harvard School of Public Health.
Jay is board certified by the American Board of Family Medicine and holds the AAFP Degree of Fellow in recognition of his service and unwavering commitment to our specialty.
He lives in Huntington Beach with his wife, pediatrician Dr. Sumina Fukami, their three children and a pandemic puppy. Jay brings not only deep experience but heart and purpose to every role he takes on, determined to build a healthier, more just future for all.
