Discordance of Perception & Relayed Communication in Medicine

This presentation will focus on the misconception of what information our EMR’s relay vs. what is actually communicated to colleagues and pharmacies during patient care, thus putting our patients at risk for medication errors. We will also discuss the elements of a thorough and consistent medication reconciliation process that ensures medication is initiated and discontinued appropriately.
